It's awesome how you drew Watson. Pardon for crashing the party. But I don't think women wouldn't be able to be doctors at that time. But at the same time she can disguise herself as a man ( a very convincing one) and work as a doctor. But Sherlock knew that and when Dr Watson retired from the military and went back as a woman, Sherlock took her in to help with cases.
